Indications
Trospi Tablet is used for:
Overactive bladder
Adult Dose
Oral
Overactive bladder
Adult: For the symptomatic treatment of urge urinary incontinence, urinary frequency and urgency:
As conventional tab: 20 mg bid.
As extended-release cap: 60 mg once daily in the morning.
Reassess the need for continued therapy at regular intervals of 3-6 months.
Elderly: >75 years As conventional tab: 20 mg once daily based on patient tolerability.
Hepatic Impairment
Use with caution
Severe: Contraindicated.

CrCl <30 mL/min: As conventional tab: 20 mg once daily or 20 mg on alternate days.
As extended-release cap: Not recommended.
CrCl>30 mL/min: Dose adjustment not provided by manufacturer; monitor for increased adverse effects
Administration
Should be taken on an empty stomach. Take 1 hr before meals or on an empty stomach.
Contra Indications
Existing or predisposition to urinary retention, gastric retention, severe gastrointestinal conditions (e.g. toxic megacolon), uncontrolled angle-closure glaucoma, myasthenia gravis, tachyarrhythmia. Severe hepatic impairment.
Precautions
Use caution in renal impairment, geriatric patients, moderate or severe hepatic dysfunction
Angioedema of the face, lips, tongue and/or larynx reported (including with 1st dose); if involvement of the tongue, hypopharynx, or larynx occurs promptly discontinue; promptly provide appropriate therapy and/or measures necessary to ensure a patent airway function
Administer with caution to patients with clinically significant bladder outflow obstruction or gastrointestinal obstructive disorders due to risk of urinary or gastric retention; use with caution in patients with conditions such as ulcerative colitis, intestinal atony, and myasthenia gravis
To administer, must carefully monitor patients with controlled narrow-angle glaucoma; benefits should outweigh risks
Somnolence reported; advise patients not to drive or operate heavy machinery until they know how therapy affects them
Drug substantially excreted by kidney; effects of moderate renal impairment on systemic exposure are not known but systemic exposure is likely increased; risk of anticholinergic adverse reactions (including dry mouth, constipation, dyspepsia, urinary tract infection, and urinary retention) is expected to be greater in patients with moderate renal impairment

Interactions
Other agents that are eliminated by active tubular secretion (e.g. procainamide, pancuronium, vancomycin, morphine, tenofovir) may increase the serum concentration of trospium chloride and/or the coadministered drug. May decrease the exposure and peak serum levels with metformin immediate-release tab.
May potentiate the effects of agents with anticholinergic action (e.g. amantadine, TCAs). May enhance the tachycardic effect of β-sympathomimetics. May decrease the efficacy of prokinetic agents (e.g. metoclopramide).

Mode of Action
Trospium chloride is a quaternary ammonium antimuscarinic agent which antagonises the effects of acetylcholine at muscarinic receptors located in the cholinergically innervated organs. It also reduces the contractile tone of smooth muscles in the urinary bladder.
