Indications
Maxitrol Eye drops is used for:
Bacterial eye infections, Ocular inflammation
Adult Dose
Ophthalmic
Ocular inflammation with suspected or confirmed bacterial infection
Adult: Eye drops: Instill 1-2 drops in the conjunctival sac(s) of the affected eye(s) up to 4-6 times daily; may be used up to hrly in severe disease. Reduce frequency once condition improves.

Contra Indications
Hypersensitivity. Epithelial herpes simplex keratitis (dentritic keratitis), vaccinia, varicella, and many other viral diseases of the cornea and conjunctiva. Hypersensitivity to a component of the medication (Hypersensitivity to the antibiotic component occurs at a higher rate than other component).
The use of these combinations is always contraindicated after uncomplicated removal of a corneal foreign body.
Precautions
Prolonged use may cause glaucoma, damage to the optic nerve, defects in visual acuity and fields of vision, posterior subcapsular cataract formation and increased risk of secondary ocular infections. Increased risk of perforations in diseases which can cause thinning of the cornea or sclera. Corticosteroids may mask infection or worsen existing infections in acute purulent conditions of the eye. Regular monitoring of the intraocular pressure is recommended if product is used for ?10 days. Long term application of topical corticosteroids may also increase the risk of ocular fungal infections. Neomycin sulfate may cause cutaneous sensitisation. Pregnancy and lactation.
Lactation: Unknown if excreted in breastmilk; minimal systemic absorption with ophthalmic administration

Dexamethasone shown to be teratogenic in mice and rabbits following topical ophthalmic application in multiples of therapeutic dose
In mouse, corticosteroids produce fetal resorptions and a specific abnormality, cleft palate; in the rabbit, corticosteroids have produced fetal resorptions and multiple abnormalities involving head, ears, limbs, palate, etc
There are no adequate or well-controlled studies in pregnant women; however, prolonged or repeated corticoid use during pregnancy associated with increased risk of intra-uterine growth retardation; neomycin and polymyxin B sulfates and dexamethasone ophthalmic suspension should be used during pregnancy only if potential benefit to mother justifies potential risk to embryo or fetus; infants born of mothers who have received substantial doses of corticosteroids during pregnancy should be observed carefully for signs of hypoadrenalism
Lactation
Systemically administered corticosteroids appear in human milk, could suppress growth, interfere with endogenous corticosteroid production, or cause other untoward effects; not known whether topical administration of corticosteroids could result in sufficient systemic absorption to produce detectable quantities in human milk; because many drugs are excreted in human milk, caution exercise caution when drug administered to a nursing woman

Mode of Action
Dexamethasone is a corticosteroid with mainly glucocorticoid activity; it is used topically mainly for its anti-inflammatory action. Neomycin is a broad-spectrum aminoglycoside antibiotic that is used topically for the treatment of infections of the skin, ear and eye due to susceptible staphylococci and other organisms. Polymyxin B is active only against gram-negative bacteria especially Pseudomonas aeruginosa.
